How to Order a Virtual Karyotype for a Clinical Sample

IT IS CRITICAL THAT A HEMATOPATHOLOGY OR SURGICAL PATHOLOGY REPORT ACCOMPANY THE SPECIMEN TO OUR LABORATORY.
This will enable proper specimen handling, a more robust virtual karyotype, a more useful clinico-pathologic correlation in the report, and simply help prevent us all from practicing medicine in a vacuum.

Hematologic Cancers (Liquid)
For hematologic specimens, the initial step in specimen processing is enrichment for the clonal population using density-based or magnetic bead methods. The immunophenotype of the clonal population will guide the enrichment. This enrichment step results in a more robust virtual karyotype and enables the detection of small subclones. DNA is extracted after enrichment. If the flow cytometry report is not available when you ship the specimen, please have your flow cytometry laboratory fax us a copy of the report. Blood, 1-5mL in EDTA (purple top), ship at room temperature.
Leukemic blood, 1-5mL in EDTA (purple top), WBC___________________, ship at room temperature.

Solid Tumors
Please attach a copy of the surgical pathology report to the requisition for virtual karyotyping. Fresh tissue will always provide a cleaner virtual karyotype, but paraffin embedded tissue is also acceptable. For paraffin embedded samples, we request ten 10um unstained slides plus one H&E. There must be at least 0.3cm of tumor present on each slide. Try to select a block that has minimal 'normal clone contamination' from inflammatory cells, stromal cells, and other non-neoplastic cells. We perform manual microdissection tumor cells prior to DNA extraction in order to enrich for the clonal population. Alternatively, you can send the paraffin block, and we will return the block after sectioning.
